Accident investigation is the process of determining the root causes of accidents, on-the-job injuries, property damage, and close calls in order to prevent them from occurring again.
This Accident investigation is a group assignment for the course of the human factor in MPD2. Our group decides to pick 2018 Yilan train derailment as our investigating accident practicing. During this assignment, we described the accident and potential contributors to the accident. We also noted that the causes of many of these accidents are still somewhat uncertain but we discussed the human factors that might have contributed to the issue. These might include display or control design, fatigue issues, motivation issues, decision-making problems, etc.
